Age-related Macular Degeneration: Poised for a New Treatment Era
For more than a decade, ophthalmologists have treated wet age-related macular degeneration (AMD) with eye injections given every month or two, and dry AMD with antioxidant vitamins. These treatments were groundbreaking when introduced, offering hope for the first time that this sight-threatening disease could be slowed, and in some cases stopped or even reversed. As we mark February as AMD Awareness Month, Acuity Eye Center Lahore Pakistan and the American Academy of Ophthalmology is highlighting what the next decade may hold for the 11 million Americans suffering from AMD.
The good news is that AMD treatment continues to evolve to the benefit of patients. Ophthalmologists expect to soon have more effective options to protect people from going legally blind from AMD.
"While our current treatments have made a huge difference in the lives of hundreds of thousands of people, new treatments offer hope to patients whose AMD previously could not be treated,” said Sunir J. Garg, MD, FACS, a retina specialist and clinical spokesperson for the American Academy of Ophthalmology. “New treatments will also help patients receive beneficial treatment more conveniently than ever before."

Dry AMD
It can be divided into three forms: early, intermediate, and late. For those with an intermediate disease, a formulation of antioxidant vitamins called the AREDS2 formula can help many patients reduce their risk of vision loss. But for those with late-stage disease, also called geographic atrophy, there is no treatment available. However, there are several promising clinical trials underway.
The Academy offers these steps to help people take control of their eye health:

1. Get regular comprehensive medical eye exams.

Because AMD often has no early warning signs, getting regular comprehensive eye exams from an ophthalmologist is critical. Academy guidelines state that adults with no signs or risk factors for eye disease get a baseline eye disease screening at age 40 — the time when early signs of disease and changes in vision may start to occur. From age 40 to 54, get your eyes examined every 2 to 4 years; from 55 to 64, every 1 to 3 years. By age 65, get an exam every one to two years, even in the absence of symptoms or eye problems. If you have risk factors for eye disease, you will need to be examined more frequently.
2. Quit smoking.

Numerous studies show smoking increases the risk of developing AMD, and the speed at which it progresses. Smokers are twice as likely to develop macular degeneration compared with a nonsmoker
Many studies demonstrate that eating a diet rich in fruits, vegetables, and nutrient-packed foods, such as salmon and nuts, may reduce the risk of AMD. Research also suggests that patients who ate fresh fish, an important source of omega-3s, were at lower risk of developing AMD.
4. Exercise regularly.
Exercising three times a week can reduce the risk of developing wet AMD by 70 per cent. Studies also show that physical activity may lower the odds of both the early and late stages of AMD.
5. Know your family’s eye health history.
If you have a close relative with AMD, you have a 50 per cent greater chance of developing the condition. Before your next eye exam, speak with your family about their eye health history. You may need more frequent eye exams based on your family history.

Next time you step outside to enjoy the summer sun, don’t forget to bring a pair of sunglasses. Most people know that the sun’s damaging ultraviolet rays are bad for the skin. But did you know that too much sun on unprotected eyes increases the risk of eye diseases? This summer, Acuity Eye Center Lahore Pakistan and the American Academy of Ophthalmology remind you that sunglasses are more than a bold fashion statement, they are a smart health choice.

Long-term exposure to the sun without proper protection can increase the risk of eye disease, including cataract, macular degeneration, growths on the eye, and a rare form of eye cancer. Even short-term exposure can damage the eyes. Sun reflecting off water can cause a painful sunburn on the front part of the eye, called photokeratitis. It causes redness, blurry vision, sensitivity to bright light, and in rare cases, even temporary vision loss.
The good news is that prevention is simple: Wear sunglasses that block 99 to 100 percent of both UVA and UVB radiation. When purchasing sunglasses, ophthalmologists – physicians who specialize in medical and surgical eye care – urge the public to choose substance over style, and consider these 6 shopping tips:
Shop labels. The single most important thing to look for when buying sunglasses is a sticker or tag indicating that they block 99 to 100 percent of both UVA and UVB radiation.
Buy oversized. The more coverage from sunglasses, the less sun damage inflicted on the eyes. Consider buying oversized glasses or wraparound-style glasses, which help cut down on UV entering the eye from the side.
Don’t be fooled by the color. While very dark lenses may look cool, they do not necessarily block more UV rays.
Consider your sport. Some sunglasses come with amber, green or gray lenses. They do not block more sun but can increase contrast, which may be useful for athletes who play sports such as baseball or golf.
Consider polarized lenses. Polarization reduces glare coming off reflective surfaces like water or pavement. This does not offer more protection from the sun but can make activities like driving or being on the water safer or more enjoyable.
Don’t break the bank! Sunglasses don't have to cost a lot of money to provide adequate eye protection. Less expensive pairs marked as 100 percent UV-blocking can be just as effective as pricier options.
If you doubt your sunglasses have the UV protection claimed by a retail tag or if they are simply old and you want to make sure, take them to an optical shop. Any shop that has a UV light meter can test your sunglasses.

“It’s important to protect your eyes year-round, not just during the summer,” said Dianna Seldomridge, MD, clinical spokesperson for the American Academy of Ophthalmology. “Harmful UV rays are present even on cloudy days. Wearing a wide-brimmed hat also cuts down on exposure. And don’t forget babies and kids. They also need to wear hats and sunglasses.”

By age 65, one in three Americans will have a vision-impairing eye disease. Many sight-robbing conditions can be effectively treated if detected early enough, in many cases limiting or eliminating the damage to eyesight. During the month of May, Acuity Eye Centre Lahore Pakistan joins the American Academy of Ophthalmology is sharing valuable information about how to take care of your vision.
Four eye diseases — age-related macular degeneration (AMD), diabetic retinopathy, glaucoma, and cataracts — account for most cases of adult blindness and low vision among people in developed countries. Because these eye diseases cause no pain and often have no early symptoms, they do not automatically prompt people to seek medical care. But a thorough checkup by an ophthalmologist — a physician who specializes in medical and surgical eye care — can detect them in their earliest stages. Early treatment is vital because it can slow or halt disease progression or, in the case of cataracts, restore normal vision.
A thorough eye exam can also detect other health conditions, such as stroke, cardiovascular disease, diabetes, high blood pressure, autoimmune diseases, sexually transmitted diseases, and some cancers. It’s not uncommon for a trip to the ophthalmologist to actually save a life.
The Academy’s global community of 33,000 physicians urges you to follow these five simple steps to take control of your eye health today: Get a comprehensive medical eye exam at age 40. Early signs of disease or changes in vision may begin at this age. An exam by an ophthalmologist is an opportunity to carefully examine the eye for diseases and conditions that may have no symptoms in the early stages. 5 Steps to Lower Your Risk of Eye Disease
Know your family history
Certain eye diseases can be inherited. If you have a close relative with macular degeneration, you have a 50 percent chance of developing this condition. A family history of glaucoma increases your glaucoma risk by four to nine times. Talk to family members about their eye conditions. It can help you and your ophthalmologist evaluate your risk.
Eat healthy foods
A diet low in fat and rich in fruits, vegetables, and whole grains, benefits the entire body, including the eyes. Eye-healthy food choices include citrus fruits, vegetable oils, nuts, whole grains, dark green leafy vegetables, and cold-water fish.
Stop smoking
Stop Smoking
Smoking increases the risk of eye diseases such as cataract and age-related macular degeneration. Smoking also raises the risk for cardiovascular diseases which can indirectly influence your eye health. Tobacco smoke, including second-hand smoke, also worsens dry eye.
Wear sunglasses

Exposure to ultraviolet UV light raises the risk of eye diseases, including cataract, fleshy growths on the eye, and cancer. Always wear a hat and sunglasses with 100 percent UV protection while outdoors.

But these advances cannot help patients whose disease is undiagnosed or patients who are unaware of the seriousness of their disease. People’s lack of understanding about AMD is a real danger to public health. A recent study showed that most people with AMD don’t realize it’s a chronic health issue that requires regular attention for the rest of their lives.

AMD is a degenerative disease that happens when part of the retina called the macula is damaged. It’s the part of the eye that delivers sharp, central vision needed to see objects straight ahead. Over time, the loss of central vision can interfere with everyday activities, such as the ability to drive, read, and see faces clearly.

Risk of AMD
Commonly named risk factors for developing macular degeneration include:
Aging. Significant vision loss accompanying more advanced forms of AMD increases from fewer than 1 percent among people in their 60s to more than 15 percent among people in their 90s, according to the Canadian Medical Association Journal (February 2004).

Obesity and inactivity. Overweight patients with macular degeneration had more than double the risk of developing advanced forms of macular degeneration compared with people of normal body weight, according to one study reported in Archives of Ophthalmology (June 2003). In the same study, those who performed vigorous activity at least three times weekly reduced their risk of developing advanced AMD, compared with inactive patients.
High blood pressure (hypertension). Investigative Ophthalmology and Vision Science reported the results of a European study demonstrating that high blood pressure may be associated with the development of macular degeneration (September 2003).
Smoking. Smoking is a major AMD risk factor and was found in one British study to be directly associated with about 25 percent of AMD cases causing severe vision loss. The British Journal of Ophthalmology in early 2006 also reported study findings showing that people living with a smoker double their risk of developing AMD.
Drug side effects. Some cases of macular degeneration can be induced from side effects of toxic drugs such as Aralen (chloroquine, an anti-malarial drug) or phenothiazine. Phenothiazine is a class of antipsychotic drugs, including brand names of Thorazine (chlorpromazine, which also is used to treat nausea, vomiting and persistent hiccups), Mellaril (thioridazine), Prolixin (fluphenazine), Trilafon (perphenazine) and Stelazine (trifluoperazine). The American Academy of Ophthalmology notes that findings regarding AMD and risk factors have been contradictory, depending on the study. The only risk factors consistently found in studies to be associated with eye disease are aging and smoking.

A recent study found that some of the top-selling products do not contain identical ingredient dosages to eye vitamin formulas proven effective in clinical trials.

Degenerative diseases, such as AMD, can now be successfully treated, but early detection is imperative to avoid lasting consequences."

How Is Macular Degeneration Treated?
Treatment of AMD
There is as yet no outright cure for age-related macular degeneration, but some treatments may delay its progression or even improve vision.
Treatments for macular degeneration depend on whether the disease is in its early-stage, dry form or in the more advanced, wet form that can lead to serious vision loss. No FDA-approved treatments exist yet for dry macular degeneration, although nutritional intervention may help prevent its progression to the wet form.
For wet AMD, treatments aimed at stopping abnormal blood vessel growth include FDA-approved drugs called Lucentis, Eylea, Macugen, and Visudyne used with Photodynamic Therapy or PDT. Lucentis has been shown to improve vision in a significant number of people with macular degeneration.

Starting your holiday meal with a salad is an excellent way to ensure that you and your guests get plenty of zeaxanthin and lutein, two nutrients that help protect your central vision. Adding kale, spinach, or romaine lettuce to salads helps your eyes absorb damaging blue light, combats the effects of cigarette smoke and pollution, and also decreases your risk of developing age-related macular degeneration (AMD), a condition that affects the macula, the part of your retina responsible for central vision. You will also find lutein in grapes, kiwis, broccoli, peas, corn, Swiss chard, and collard greens.
Fruit
Fruits high in vitamin C, such as strawberries and oranges, also offer important vision benefits. Vitamin C is an antioxidant, a substance that can prevent cell damage caused by free radicals. Eating vitamin C-rich foods helps keep the collagen in your cornea healthy and reduces the risk of cataracts and AMD. In addition to strawberries and oranges, you will also find vitamin C in grapefruit, kiwi, blueberries, peas, broccoli, and tomatoes.
Nuts and legumes
Nuts are also rich in omega-3 fatty acids. Nuts also contain a high level of vitamin E, which can protect the eye from age-related damage.

Carrots
Carrots are rich in both Vitamin A and beta carotene. Beta carotene gives carrots their orange color.
Vitamin A plays an essential role in vision. It is a component of a protein called rhodopsin, which helps the retina to absorb light.
Research on beta carotene's role in vision is mixed, though the body needs this nutrient to make vitamin A.
Eggs
The vitamins and nutrients in eggs, including lutein and vitamin A (which may protect against night blindness and dry eyes), promote eye health and function.

Since you are in 40s or 50s, you probably noticed that your vision is changing. Perhaps you need glasses to see up close or you have more trouble adjusting to glare or distinguishing some colors. These changes are a normal part of ageing process. These changes alone cannot stop you from enjoying an active lifestyle or stop you from maintaining your independence. In fact, you can live an active life well into your golden years without ever experiencing severe vision loss. But as you age, you are at higher risk of developing age-related eye diseases and conditions. These include: age-related macular degeneration, cataract, diabetic eye disease, glaucoma, low vision and dry eye.
Get a comprehensive dilated eye exam.
Everyone age 50 or older should visit an eye care professional for a comprehensive dilated eye exam. Many eye diseases have no early warning signs or symptoms, but a dilated exam can detect eye diseases in their early stages before vision loss occurs. Early detection and treatment can help you save your sight. Even if you aren’t experiencing any vision problems, visit your eye care professional for a dilated eye exam. He or she will tell you how often you need to have one depending on your specific risk factors.

Most Americans Unaware of One of the Leading Causes of Blindness Among Seniors
ACUITY EYE CENTER LAHORE PAKISTAN and the American Academy of Ophthalmology reminds the public that a simple eye exam is key to saving sight against Age-Related Macular Degeneration
<LAHORE, PUNJAB> – 09-FEB-2019 — Age-Related Macular Degeneration is one of the leading causes of blindness among seniors, affecting approximately 2.1 million people nationwide. By 2050, it is expected that the number will more than double to 5.4 million. People may be putting themselves at unnecessary risk of vision loss by neglecting to have sight-saving eye exams. Throughout February, ACUITY EYE CENTER LAHORE PAKISTAN joins the American Academy of Ophthalmology in emphasising AMD awareness and encouraging those who are most at risk to ensure the health of their eyes by getting an eye exam from an ophthalmologist – a physician who specializes in the medical and surgical treatment of eye diseases and conditions.

AMD is a degenerative disease that damages the retina, which is the light-sensitive tissue at the back of the eye that focuses images and relays information to the brain. Over time, retinal damage can lead to permanent loss of central vision, limiting the ability to drive, read and recognize faces.
Age Related Macular Degeneration
There are two forms AMD – wet and dry. While the dry form of AMD leads to gradual vision loss, the wet form progresses at a faster rate and is responsible for 90 percent of all AMD-related blindness. Recent advancements in treatment options have significantly decreased the incidence of blindness. However, it is critical to get diagnosed and begin treatment as soon as possible to protect vision.
The leading cause of blindness in seniors is age-related macular degeneration. If you have a close relative with AMD, you have a 50 percent chance of developing the condition. Learn other risk factors and the steps you can take to protect yourself from AMD-related blindness:
The Academy recommends the following steps to help potentially avoid AMD and other eye diseases:
· Get regular comprehensive medical eye exams. AMD often has no early warning signs, so getting regular comprehensive eye exams from an ophthalmologist is critical to diagnosing and treating the eye disease in its early stages. The Academy recommends that people over age 65 get an exam every one to two years, even if they have no symptoms of eye problems.

· Quit smoking. Numerous studies have shown smoking to increase the risk of developing AMD and the speed at which it progresses. If you smoke, you are twice as likely to develop macular degeneration compared with a nonsmoker.
· Know your family’s eye health history. If you have a close relative with AMD, you have a 50 percent greater chance of developing the condition. Before you go in for your next eye exam, speak with your family about their eye health history. Sharing this information with your ophthalmologist may prompt him or her to recommend more frequent eye exams. The earlier AMD is caught, the better chances you may have of saving your vision.
· Eat a diet rich in omega-3s and low in cholesterol and saturated fat. Several studies have shown that people who had a reduced risk of AMD had diets rich in omega-3 fatty acids, such as fish. In one study of patients who were at moderate risk for AMD progression, those who reported the highest omega-3 intake (not in the form of a supplement) were 30 percent less likely to develop advanced AMD after 12 years. In another study, an increased risk of AMD was found in individuals who had a higher intake of saturated fats and cholesterol and in those with a higher body mass index.
· Exercise regularly. Many studies have shown that getting regular exercise can benefit your eyes. One study found that exercising three times a week reduced the risk of developing wet AMD over 15 years by 70 percent.
“Most people understand the importance of annual medical examinations," said Rahul N. Khurana, M.D., a clinical spokesperson for the American Academy of Ophthalmology. “However, we often forget that our eyes also need regular evaluation by a medical doctor. Degenerative diseases, such AMD, can now be successfully treated, but early detection is imperative to avoid lasting consequences."
